Fortunately, we have continued to make swift and significant progress largely due to the many technological advancements made in recent years. With the proper medical equipment in the right physical environment and the very best doctors at the helm, our knowledge of curative healthcare and our ability to treat illnesses is evolving by leaps and bounds. A growing number of patients have access to state-of-the-art hospitals, which has significantly increased their chances of recovery. This recovery, however, depends on several success factors. The first defining factor is the availability of duly qualified doctors. The second is the availability of the best medical equipment, housed in temperature-regulated rooms with the proper finish and the ergonomic dimensions. In order to function properly, hospitals must be efficiently designed with the shortest possible walking distance between departments. Hundreds of hours can be gained each day in hospitals with an efficient layout. In terms of financial gains, efficient design can make the difference between a profitable and an unprofitable hospital. The same is true when processing the enormous influx of patients: by leading them directly to their destinations and keeping their stay as short as possible, the hospital can increase capacity. The result is a more profitable and cleaner hospital with a reduced risk of errors. More importantly, this leads to a better environment for patients and staff. A pleasant environment like this lowers the patient’s stress level and increases his chances of a swift recovery. University Hospital Maastricht is the youngest of its kind in the Netherlands. The original plans date from 1968, but the new hospital was only completed in 1990. It has continued to expand since then and currently employs some 6,000 people. The hospital is now part of the Maastricht Health Campus, a collaboration between the faculties of Health, Medicine and Life Sciences and Psychology and Neuroscience at Maastricht University, the province of Limburg and some 40 companies. Treatment, nursing, research and education comprise the core tasks of University Hospital Maastricht. In addition to offering standard patient care, the hospital also offers specialised clinical and referral care. These have been aligned with fundamental and experimental and other clinical research, which have manifested in four key clinical areas: cardiovascular disease, chronic illness and oncology, mental healthcare and neuroscience, and primary care. Holland Hospital Architects designed the new hospital, which was constructed relatively quickly on the outskirts of Maastricht. In terms of size and scale, this building transcends the structural limits of the city, giving the impression that a spaceship has landed in Maastricht. The complex also functions as a city within a city; a company that is fully operational 24 hours a day. Due to its immense size and functionality, the complex was designed as an orthogonal city with covered streets and squares. The complex nature of the hospital is reflected in clearly demarcated zones for nursing, treatment, research and education. For logistical continuity, the design incorporates a clear network of streets and squares. The squares contain the building’s vertical elevation points, such as stairs and elevators, and were designed to facilitate personal interaction. For patients and visitors, these central meeting points offer an area for reflection; for students and staff they become a hub for knowledge exchange. It is essential that not only the patients and visitors, but also the staff should feel comfortable in the building complex. The organisation’s ability to operate at a leading scientific level rests on its ability to retain highly trained and qualified staff. This building also houses skills labs the students can use to hone their practical skills and competencies. Perhaps the most important component of the academic building is the meeting centre where students and professors can exchange ideas and hold scientific discussions while sharing a meal. Very early on, students have access to a scientific environment in which interaction, stimulation and interdisciplinary knowledge exchange take centre stage. Health Campus The next step for the university hospital is to create a health campus. Excellent research, education and clinical care will be reflected in several innovative and ambitious plans where researchers, companies and consumers converge. Concrete projects are now possible thanks to widespread support, shared ambitions and a lot of perseverance. The Particle Therapy Centre, Living Lab Limburg (discussed in more detail in this magazine) and the Cardiovascular Centre are just a few examples of projects currently under development. Each new project serves to strengthen the foundations of the Maastricht Health Campus. gerçekleşir. He is the head of the Cornea Clinic at the Academic Centre for Refractive Surgery (ACRC) at the UHM and medical director of the Surgical Day Clinic at the UHM. Nuijts earned his medical degree from the University of Limburg in 1986. After serving as an ophthalmology assistant at the Dr A. Mathijsen Military Hospital in Utrecht (19861988), he began training as a certified ophthalmologist at the Academic Medical Centre (AMC) in Amsterdam. Before and after obtaining his degree, he underwent research fellowships in corneal surgery in the United States. He started a corneal surgical fellowship at the Ophthalmological Hospital in Rotterdam and was awarded a degree in 1995 for his research on the ocular toxicity of ophthalmological fluids and medications used during surgery. Interview Mülakat In terms of administrative qualifications, Nuijts is chair of the Dutch Intraocular Implant Club (NIOIC, the association of cataract surgeons in the Netherlands), treasurer of the European Society of Cataract and Refractive Surgeons (ESCRS), treasurer of the Cornea Workgroup at the Dutch Ophthalmic Society (NOG), and sits on the boards of both the European Cornea Society (EuCornea) and the Dutch Transplant Society (NTS). He has authored and co-authored over a hundred scientific publications and delivered over 200 presentations in the Netherlands and abroad. His research group investigates new treatment methods for corneal transplants and keratoconus. They also conduct research on new artificial lenses at the Maastricht UMC Ophthalmology Clinic, such as toric lenses to correct cylinder deviations and multifocal lenses to replace traditional reading glasses. These new lenses are used in refractive surgery and cataract surgery. Innovative treatment methods like these strive to improve a patient’s results following corneal transplant surgery and cataract surgery. Dr. Rudy Nuijts 14 Holland Hospital Architects Articles & Interviews Hollanda Hastane Mimarları Makaleler & Mülakatlar W hat inspired the development of an eye clinic? “Ophthalmology is a specialisation that makes little to no use of a hospital’s primary services, such as labs and radiology centres. Ophthalmologists do, however, use dozens of diagnostic tools of no relevance to other medical specialities. This means we can function as a relatively independent unit outside the hospital’s main organisation. In recent decades, the number of ophthalmology patients has significantly increased along with the available diagnostic and therapeutic possibilities. We have also seen a shift from clinical operations to outpatient operations and from general anaesthesia to local anaesthesia. The Maastricht Eye Clinic has acquired several new practices over the years. In 1994, ophthalmologists from the UHM created an external facility to meet the growing demand for cataract surgery. The huge risk (financial and otherwise) borne by these doctors contributed to the emergence of a strong entrepreneurial spirit. For years there has been a dedicated focus on redesigning care practices to maximise quality and turnaround time. The best way to achieve and maintain high-quality processes is to become independent from other organisational units; to become a clinic.” What is the underlying vision of the eye clinic? “The ultimate opportunity to create an eye clinic presented itself when the Board of Directors decided to expand the UHM with two new constructions: the towers. It soon became apparent that all seven floors of just one of these were necessary to ensure the facility could handle the growing number of patients in the future. All facilities, both diagnostic and therapeutic, including four operating rooms, were housed in the Eye Tower. What made this so unique was that the Board of Directors imposed absolutely no restrictions on the spatial layout of the building, which meant it could be fully tailored to the needs of our ophthalmologists. In close collaboration with the architects, these doctors could decide where to position each floor and how to tailor its design to meet its purpose. The results are more than satisfactory because the doctors knew exactly what they wanted and the architects knew exactly how to translate this into architectural terms.” How does the building reinforce these care processes? “The care processes have been designed in a way that reduces the time a patient spends in the building to one hour on average. This includes all diagnostic tests, the doctor’s consult and a relatively long wait due to the drugs used to dilate the pupil. We’ve also reached the point where the influx of surgical patients is equal to the outflow. In other words: the patient is immediately scheduled for surgery and followup consults. Emergency surgery can even be performed the same day the patient reports to the clinic with a complaint. It’s important that the building has the flexibility to allow us to gain new scientific insights and immediately implement new technological developments.” How have your patients responded to the clinic? “The patients are generally very satisfied with us, especially those who experienced the old situation. The employees have a pleasant work environment and all the room they need to perform their tasks thanks to a strict separation between the different levels, from front office to back office.” G öz kliniği nasıl oluştu? “Göz hastalıkları bölünmü hastanedeki, laboratuvar ve radyoloji gibi merkezi hizmetleri hemen hemen hiç kullanmayan bir uzmanlık alanıdır. This organisation underwent an extensive transformation to ensure that this care concept could be implemented and translated into efficient architectural design. The trick was to design a building that not only focused on the patient’s health and treatment, but also on a general sense of wellbeing. The organisation The hospital has been thematically divided into four functional areas: public, meeting, staying and working. Public The building’s fundamental concept is based on a linear structure with the covered atrium as its axis. This atrium is bordered by the public and patient facilities, including the entrances, the reception areas, the catering facilities, parking access, waiting rooms and the consultation centre. Meeting The meeting centre consists of standard consultation, research and treatment rooms. The doctors and support staff meet the patient in the research room. The care process is organised entirely around the patient. Staying All patients staying here have a private room, a private bathroom and a sofa bed for their guests. They can use the integrated bed controller to place meal orders and to close or open the curtains or the door to their room: hypermodern convenience at an affordable price. A self-reliant patient with access to state-of-the-art technology helps curb staffing costs. In a traditional hospital setting, these costs amount to roughly 65% of the budget. Integrated technology and new processes can reduce this figure to less than 50% – a worthwhile solution for the growing shortage of healthcare professionals. Working The goal here is to maximise the interdisciplinary knowledge exchange between researchers and medical staff by creating an open work area with compartmentalised workstations, cockpits, lounge areas and meeting rooms. The design encourages a distinct separation of traffic flows; between the nursing and treatment units and general hospital traffic. Doctors and nurses have their own lifts, which are also used by the robotic carts used for internal transportation. The electronic patient management system has been fully integrated into the primary care process. As a result, information can be accessed anytime and anywhere. Recent technological advances, changing work processes, new collaboration techniques and business models have been translated into a new hospital design that supports the organisation and helps guide these processes. Henny van Laarhoven Henny van Laarhoven has been responsible for the realisation of the vision and concept of the new hospital which focuses on delivering ‘care in dialogue’ with the patients, creating a total chain of care, including links with the primary sector, diagnosis, treatment, rehabilitation, and relations with third party providers of care and services. The introduction of the so-called “New Way of Working” (NMvW) forms the leading strategy concept for all departments of Orbis Medical Park. The NMvW contains a highly standardised (patient) logistic concept, which is maximal supported by the most modern information technology and (automatised) logistic concepts. This standardisation is also translated in the new hospital and adjacent buildings, having only private patient rooms, standardised consulting rooms and offices with different functions and flexible working places. From june 2009 untill the end of 2010 Henny van Laarhoven was director of the supporting departments (ICT, Facilities, Administration and Property management). She implemented the Shared Service Concept in order to be able to facilitate further cooperations with other hospitals and private partners. With its functionality, flexibility and ICT infrastructure, the new building fully supports the Orbis care concept. All patients are given a private room and receive maximum support to encourage self-reliance. They have plenty of privacy, excellent communication tools and can even invite a friend or relative to spend the night. We asked ourselves the following question: What do patients want in 2010 and what do they want in the future? We also wanted to provide the best possible working conditions for our 2500 employees. Other important considerations were to ensure that patient information was available 24 hours per day from any location, to safeguard the quality of care, to adapt the environment to meet the patient’s needs and to become a leader in healthcare. These Ten Commandments, as we called them, were continually at the back of our minds during the entire process of designing, constructing and completing the new building. Aligning our treatment, screening, diagnostic and clinical and outpatient process as efficiently as possible allowed us to standardise the floor plans, medical devices and designs in the various centres. The result was a decrease in investments and an increase in patient safety because all staff members were now working with the same equipment and had access to all data (including research and treatment data) through electronic patient records. This information has been structured in a similar way for everyone. This results in huge time savings for the patients – after an examination, the results are immediately documented in the EPR. An extensive digital network was created that allows employees to access all available medical information, such as ECGs, X-rays and test results, from any computer in the building. Daylight can also be an excellent form of therapy, particularly for this sick and the elderly. Modern hospitals and nursing homes, where natural light is a scare commodity, are starting to experiment with dynamic forms of artificial lighting. They are attempting to recreate the natural light we experience throughout the day, from early morning to late evening. These lighting systems mimic our circadian rhythm, thereby reducing our stress levels. While this artificial solution, which has been substantiated by scientific research, certainly benefits the healing process, nothing beats the real deal! And certainly not when paired with a great view. The more daylight and the better the view, the less likely patients will feel trapped or confined. The key problem: patients who no longer require specialised medical attention occupying hospital beds intended for patients who do. This typically concerns elderly patients (particularly those suffering from dementia) and patients recovering from surgery. For this group of patients, the hospital is no longer the most appropriate environment. In our opinion, an excellent solution would be to erect a building near the hospital that is specifically designed for recovery. The type of care provided here would be far less intensive and the building would be far more simplistic in design – particularly in terms of medical equipment, staffing and building requirements. After all, these patients no longer require treatment in a high-quality and high-priced hospital environment. We have devoted ourselves to finding a solution for this bed management problem. Instead of expanding the number of clinical beds, we advise hospitals to consider whether developing a secondary care facility might not be a more sensible choice. Separate nursing units near or with a direct link to existing hospitals may be better able to accommodate this patient group. An advantage for the patients is that recovery in an environment like this would take place in small groups, which raises motivation levels. Palliative nursing care, a nurturing environment and a strong emphasis on personal attention serve to enhance quality of life and inspire a sense of security. A facility like this also allows for self-sufficiency, independence and freedom of choice. After all, the level of care provided in an environment like this is fundamentally different from that in a traditional hospital setting. This is the first in a series of buildings that will form the new Maastricht Health Campus. The completed building will serve as a magnet for the further development of the entire site. Given the purpose of Living Lab Limburg, this is an entirely realistic expectation. The living lab is a new phenomenon that is generating an unprecedented level of international attention. Living Lab Limburg targets two specific healthcare fields: nutrition and exercise. Whereas before, scientists, producers and consumers were inclined to focus on new needs and developments entirely independently of one another, Living Lab Limburg will unify them by providing an inspirational platform where all three parties can meet, ask questions and exchange scientific knowledge. It is, in fact, a group of ‘knowledge workshops’ where creative scientists, producers and consumers have the opportunity to share their experiences and discoveries in an innovative process. On-site research will be conducted by experts from various disciplines, with the consumers serving as their willing ‘guinea pigs’. The goal is the help scientists identify what it is they should be researching, to help producers translate fundamental knowledge into products and valuable production processes and to help consumers vocalise their need for useful and affordable ergonomic products. The building, situated at the heart of the campus, should become the link between inside and out and possess the necessary qualities to inspire the further development of the entire area. By establishing itself as an architectural icon, the building will undoubtedly serve this key purpose. The flow of incoming visitors should be funnelled directly to this building. This will not be accomplished with an austere and orthogonal building that stretches the boundaries of the building site. Instead, the ground-level façade should guide visitors toward the entrance and then guide them – from the nearby train station, among other locations – onwards. An ‘intimate’ building like this lends itself perfectly to this concept. A spiral with no floors The empty square formed by the building site itself lends itself to a multitude of architectural interpretations, all of which befitting the desired programme. The construction plan and building codes, however, do not allow for a design that would result in the new building becoming as tall as the adjacent hospital building. One option is to position the floors on the periphery of this square to create a large atrium, which would easily allow for future extensions on the inside. In terms of atmosphere, the Living Lab should be interactive, integrated and actually intimate – common traits of the creative breeding ground. Doing so would require a cohesive floor plan that 54 creates a sense of intimacy and allows for the remaining outdoor space to be utilised for future extensions, while fulfilling a key role in terms of urban planning. To encourage interaction, the building should not be constructed with traditional storeys. Instead, all floors would be tiered and connected by a central mezzanine. Traditional storeys would only minimise the continuity and fluidity of the available floor space, making it harder for people to meet and interact. A spiral-like design, however, would reinforce the continuum in the building and make it easier to convene and collaborate. Impression The photo impressions reveal that Living Lab Limburg, with its iconic appearance and its adaptable outdoor space, will become a solid and immovable anchor in an amorphous environment. With its distinct shape and special function, it will undoubtedly inspire the further development of the Maastricht Health Campus. Doing so successfully, however, would mean unifying the outdoor space with the interior design. Creating parks and squares will give the area a valuable upgrade. The building has its own bus stop and several taxi stands, which allows for true door-to-door service. The north entrance of the LLL (which is also the north entrance of the UHM) is roughly two meters higher than the main entrance on the Forum side and will feature graceful stairs with broadly spaced steps. Due to the physical nature of the site, these stairs will drop a meter or so deeper on the south side. This area lends itself perfectly to the construction of warehouses on this side that can be serviced from the hospital grounds. The entrance of the LLL is directly opposite the newly renovated entrance to the MECC-congresscentre. Latest developments Yeni gelişmeler The Haga Academic Hospital is a specialised clinical institute in The Hague with three locations. It is one of eleven trauma centres in the Netherlands and has its own Mobile Medical Team. The hospital’s main building on Leyweg, built in 1972, will be renovated and made nearly twice as large over the next few years to include new outpatient clinics, operating theatres and a maternity ward. The new main entrance, with a restaurant, café and shops, will be located on the north side, giving way onto a street that will connect the old building and the new extension. The result: a large yet compact, and therefore more efficient, hospital. The newly constructed section of the HagaZiekenhuis lobby and reception area in The Hague was officially opened on 3 June, followed by the bordering restaurant on 10 June. The lobby building now consists of several mezzanine floors that house an espresso bar, a restaurant and an auditorium with foyer. The spacious restaurant leads to the espresso bar and the lobby and has been designed to match the 160-metre indoor boulevard, which has yet to be completed, in terms of both atmosphere and appearance.
